Die Okzipitale Nervenstimulation (okzipital von Occiput = Hinterhaupt, zum Hinterkopf hin gelegen, engl. Occipital nerve stimulation, ONS) ist die elektrische Stimulation des Nervus occipitalis major (Hinterhauptnerv) mittels im Nackenbereich implantierter Elektroden zur vorbeugenden Behandlung. Diese noch experimentelle Methode wird bei Patienten erprobt, denen mit medikamentöser Vorbeugung nicht geholfen werden kann. Eine Wirkung tritt nach den bisherigen Erfahrungen nur bei einem Teil der Patienten und erst ca. zwei Monate nach der Implantation ein.[1][2]

Ergebnisse aus einer Studie mit 14 Teilnehmern:[3] Während der Beobachtungszeit von vier bis 35 Monaten empfanden drei der 14 Patienten eine Verbesserung ihrer Cluster-Kopfschmerzen von 90% oder besser. Je ein Patient berichtete eine Verbesserung von 40%, 50% und 60%. Vier Patienten hatten eine Verbesserung von 20 – 30%. Weitere vier Patienten hatten nach einer Beobachtungszeit von 4, 19, 31 bzw. 35 Monaten keinerlei Verbesserung. Zusammenfassend hat die Okzipitale Nervenstimulation in dieser Studie also nur bei fünf von 14 Patienten (36%) eine Verbesserung von 50% oder besser erbracht, nur bei drei Patienten (21%) ist ein Erfolg von mehr als 90% zu verzeichnen. Elf der 14 Patienten (79%) würden die ONS Behandlung weiterempfehlen, darunter zwei der Patienten, bei denen die Behandlung völlig erfolglos war. Der Impulsgenerator wird bei einer späteren Operation unter der Haut der Brustmuskulatur oder am Oberbauch eingesetzt.
